Continuing Care
Aftercare is an essential part of the recovery experience at Wellness Resource Center. Given the high rate of relapse at most addiction treatment facilities, we believe treatment for substance abuse is just the beginning of the recovery process.
We offer our patients consistent, continuous care from the time of admission until the patient leaves our care, and beyond. During treatment, clients meet with their therapeutic team once a week to discuss their progress and develop an aftercare and relapse prevention plan. 
When clinically appropriate, the client will begin a gradual transition into the community with the ongoing support of our staff. During this transition, we help our patients find housing and jobs, develop a budget and manage their money, and continue on with 12-step meetings and individual and group therapies.
We also offer flexible arrangements in which patients can gradually reduce the level of care they receive while retaining enough supports to prevent relapse. For example, patients can decrease their program participation from full-time to half-time or begin working a part-time job while continuing to live in our residential apartments. We also can offer recommendations for other halfway houses within the Delray Beach area or in the patient’s local area.
Clients who have graduated from full-time addiction treatment are invited to attend an aftercare meeting once a week to discuss daily life in recovery from substance abuse. Our alumni also come back once a month to socialize and share their experiences, strength, and hope with current clients. Each spring, we invite all of our alumni to a group picnic. Even after treatment ends, we encourage clients to treat our community as a safe haven they can return to for support during tough times, keeping the connection open as long each patient wants or needs.
All graduates of Wellness Resource Center are urged to attend the weekly aftercare meeting as well as the monthly alumni meeting. If a client remains in the area, ongoing supervision by a case manager is provided free of charge, and each client is urged to continue periodic contact with their primary therapist. Upon discharge, we recommend that all clients maintain ongoing psychiatric care and chemical dependency support through a provider of their choice or through one of our referrals.
